CMS Health Outcomes Survey (HOS)
is live: Help improve patient health

The CMS Health Outcomes Survey (HOS) is now live until November. This important survey helps evaluate each member's overall health and quality of life. As a provider, you can support HOS performance and patient health by addressing important health topics, like balance or walking concerns, ways to stay active, bladder control issues, and mental health awareness.

Review the 3 Star measures to prevent readmissions

Preventing hospital readmissions and ensuring follow-up care are critical for patient outcomes and our Star ratings. Focus on these three measures and the guides to learn more:

Plan All-Cause Readmissions (PCR):

For patients 18+ who had an acute inpatient or observation stay, followed by an unplanned readmission within 30 days.

Follow-Up After ER Visit (FMC):

 

For patients 18+ with high-risk chronic conditions who visited the ER.

For patients 18+ discharged from an inpatient facility.

Tips to improve these measures:

  • Schedule follow-up appointments within 7 days of discharge, or sooner if needed
  • Reserve appointment slots for recently discharged patients
  • Develop a process to track and monitor post-discharge follow-ups
  • Encourage regular PCP visits to manage chronic conditions
  • Advise patients to contact their PCP for changes in their condition
  • Submit claims promptly, ensuring the correct diagnosis and treatment codes are included

Arizona: New musculoskeletal (MSK) Care service

We’ve partnered with HOPCo Specialty Care Network (HSCN) to provide high-quality musculoskeletal (MSK) care for our Arizona members. This service allows patients to receive expert orthopedic care at home through HSCN’s digital clinic.

 

You can refer Devoted Health patients with back, shoulder, or knee pain to the HOPCo Specialty Care Network for assessment and care management. Your patients will receive personalized, evidence-based care from expert MSK clinicians. After the initial assessment, they will follow treatment in the digital clinic or, if needed, be referred to MSK specialists within the hSCN and Devoted Health network. You'll receive updates throughout their care journey.

Review the risk adjustment documentation: Cancer, morbid obesity, diabetes

Accurate documentation is crucial for risk adjustment.

  • For cancer, document as active for known presence of malignancy or active treatment, and transition to historical status once excised or eradicated. Document and code remission status for leukemia, lymphoma, and multiple myeloma.
  • For morbid obesity, link comorbidities to the weight diagnosis and ensure BMI codes are paired with the relevant weight diagnosis.
  • For diabetes, specify complications and link them to diabetes in the documentation, including a clear treatment plan for both.
